155.11 PERSONNEL PERFORMANCE EVALUATION.
   (a)   A performance evaluation system is hereby established. Each employee, unless exempted by the terms of this section, shall be evaluated by his or her immediate supervisor.
   (b)   Performance evaluations hereunder shall be conducted quarterly and completed in duplicate. The original shall be forwarded to the Director of Finance for filing in each employee's personnel file. The second copy shall be retained by the immediate supervisor.
   (c)   Performance evaluations shall become a permanent part of the personnel file of each employee. Such ratings shall provide a profile of his or her qualifications and suitability and assignment for promotions.
   (d)   Upon completion of performance evaluations the supervisor shall review such evaluations with the employee, who shall sign the form thereby indicating such review. Upon request each employee shall be provided a copy of the evaluation form and each employee may also submit a written response to his or her evaluation.
   (e)   The provisions of this section shall not apply to elected officials, officials for whose appointment approval is required by Council, or officials or employees covered by collective bargaining agreements.
