§ 94.03 APPLICATION FOR PERMIT; FILING FEE AND OTHER CHARGES.
   (A)   Generally. A person seeking issuance of a parade permit shall file an application with the Police Chief on forms provided by the city at the Police Department.
   (B)   Specifically.
      (1)   Filing period. An application for a parade permit shall be filed with the Police Chief not less than ten days or more than 180 days before the proposed parade date. An application which includes a request to close any portion of the state highway system must be filed not less than 30 days before the proposed parade date.
      (2)   Filing fee. The application for a parade permit shall be submitted with a nonrefundable fee in the amount of $10 to cover processing costs.
      (3)   Contents. The application for a parade permit shall set forth the following information:
         (a)   The name, address, and telephone number of the person seeking to conduct the parade;
         (b)   If the parade is to be held for or by an organization, the name, address, and telephone number of the headquarters of the organization, and the name and address of the authorized head of the organization;
         (c)   If the parade is to be held by or on behalf of any person other than the applicant, the applicant shall file a written statement from that other person showing authority to make the application;
         (d)   The name, address, and telephone number of the person who will be the parade chair and who will be responsible for its conduct;
         (e)   The date when the parade is to be conducted;
         (f)   The time when the parade will start and terminate;
         (g)   The location by streets of the proposed parade route, the initial starting point, and the disbanding area for the parade. If the starting point and/or disbanding areas are located on private property, the applicant shall submit written permission for the holding of the activity from the owner of the property or his or her authorized representative;
         (h)   A statement as to whether the parade will occupy all or only a portion of the width of the streets proposed to be used;
         (i)   The time at which units of the parade will arrive at the assembly area;
         (j)   The interval of space to be maintained between units in the parade;
         (k)   The approximate number of units and participants in the parade;
         (l)   The maximum length of the parade in miles, blocks, or fractions thereof;
         (m)   Any sanitation facilities which may be needed to keep the parade route and assembly areas clean and free from debris;
         (n)   A description of any public facilities or equipment needed for the parade; and
         (o)   Any other information which the Police Chief shall find necessary under the standards for issuance set forth in § 94.04.
      (4)   Late applications. Where good cause is shown and subject to meeting the standards promulgated in § 94.04, the Police Chief or the City Manager may consider any application filed after the deadline prescribed in division (B)(1) above.
      (5)   Charge for police assistance. In addition to the $10 permit fee, the city may charge the applicant $25 per hour for each off-duty police officer who is assigned by the Police Chief to regulate traffic along the parade route or otherwise render assistance for the parade.
