933.05 APPLICATIONS AND RENEWAL APPLICATIONS.
   Prior to the issuance of the commercial refuse hauler's license, the following documents must be on file in the office of the Director of Public Service:
   (a)    Each commercial hauler shall secure a policy of insurance, to be approved as to form by the City Attorney, insuring against the liability of such commercial hauler in operating his motor vehicles and for hazards arising from his business operations which do not involve vehicle exposure, in the amount of fifty thousand dollars ($50,000) for each person injured and three hundred thousand dollars ($300,000) for each accident, and one hundred thousand dollars ($100,000) property damage for each accident. Copies of certificates of insurance along with paid receipts showing such insurance coverage to be in force and effect must be deposited by each commercial hauler with the Director of Public Service. This policy shall also provide that it will not be cancelled without prior notification to the City.
   (b)    A vehicle inspection report completed by the City Police Department.
   (c)    The applicant will be required to maintain adequate Workmen's Compensation Insurance for hired employees. In order to comply with this requirement, the applicant shall furnish a current Workmen's Compensation certificate signed by the Ohio Industrial Commission showing that the applicant has paid his industrial insurance premium. Further, the applicant shall furnish each subsequent certificate to the office of the Director of Public Service.
   Application to renew a commercial refuse license must be made not less than one calendar week prior to the expiration date of the current license. Truck inspection is also required prior for refuse license renewal.
