907.02 PERMIT FOR OPENING PUBLIC WAY.

   (a)    Any person, firm or corporation desiring to obtain a permit for the purpose of opening any public street, roadway or alley shall first make application to the Service Director and shall fill out fully and completely the application form provided, which form shall include the property owner's and contractor's name and address and the telephone number of each, the street, street number and lot number of the location of such installation.

   (b)    Each permit shall be made in triplicate, each bearing the same numerals, and when completed, the original thereof shall be given to the applicant and the duplicate shall be retained by the Service Director, as the City record, and the triplicate copy shall be forwarded to the Auditor.
