§ 153.36 APPLICATION REQUIREMENTS. 
   (A)   Application for a development permit shall be made on forms furnished by the Building Official and shall include, but be not limited to, three black and white prints of a plot plan of the premises, drawn to scale and certified by a licensed land surveyor or civil engineer, showing the actual shape and dimensions of the lot, the size and location of all existing and proposed structures and land uses, the layout of parking and loading facilities, the location of outdoor storage areas, existing and proposed grades, the location of existing and proposed drainage facilities and utilities, base flood elevation data and limits of the floodplain area.
   (B)   Specifically, the following information is required:
      (1)   Elevation in relation to mean sea level of the lowest floor (including basement) of all structures;
      (2)   Elevation in relation to mean sea level to which any structure has been flood-proofed;
      (3)   Where a nonresidential structure is intended to be made watertight below the base flood level a registered professional engineer or architect shall develop and/or review structural design specifications, and plans for the construction, and shall certify that the design and methods of construction are in accordance with acceptable standards of practice for meeting applicable provisions of Federal Management Agency, § 60.3(c)(4), revised effective January 1, 1986. (Specifically, § (c)(3)(ii) or § (8)(ii)). A record of such certificates which includes the specific elevation (in relation to mean sea level) to which the structures are flood-proofed shall be maintained by the city pursuant to § 52.22(a)(9)(iii) of the effective Federal Emergency Management Agency provision; and
      (4)   Description of the extent to which any watercourse will be altered or relocated as a result of proposed development.
