(a) The following information shall be provided by a person, firm or corporation applying for the snow plow operator's permit required by Section 757.01:
(1) Name, home address, business address, state driver's license number and telephone number;
(2) Make, model, serial number and state license plate number of the vehicle or machine to be used for snow plowing and removal.
(b) The application for a snow plow operator's permit shall be accompanied by a nonrefundable fee of five dollars ($5.00) for each vehicle registered.
