Application for a license required by Section 880.01 shall be made to the Police Chief on a form prescribed therefor. When applying, the applicant shall state his or her name, residence, business address, age and occupation, the name and business address of the person by whom employed, or to be employed, the length of service with the employer, the kind of employment of the applicant during the previous year, and the nature, character or description of the books, magazines or literature for which subscriptions are solicited or to be supplied, along with a personal description of the applicant, the length of time for which the license is desired (provided that the same shall not exceed one year), fingerprints of the applicant, an affidavit of the applicant as to the truth of his or her application, and other information required by the Police Chief. The application shall be accompanied by adequate credentials or other evidence of good moral character and identity of the applicant as the Police Chief may require.
