   DIRECTOR OF PUBLIC SERVICES CERTIFICATION STATEMENT

   The undersigned certifies that the applicant of the final plat for the above named subdivision:

   Has demonstrated compliance with all conditions set forth by the City Planning Commission in approval of the preliminary plat,

   That the subdivision improvements have been designed by a professional engineer registered in the State of Ohio,

   That all improvements have been constructed in accordance with the City of Wadsworth's Engineering Rules and Regulations or a financial guarantee for the completion of said improvements has been filed with the Engineering Department,

   All cash amounts due have been paid to the City of Wadsworth,

   As-built drawings on mylar for all improvements and stormwater detention basin volume verification have been accepted by the Engineering Department, and

   All engineering plan review fees and field inspection fees have been paid.
