The application for the permit required by Section 751.03 shall contain all of the following:
(a) The name and residence address of the applicant;
(b) The address where the personal property is to be offered for sale;
(c) The dates on which the sale shall be conducted;
(d) A general description of all the tangible property to be included in the sale;
(e) A statement as to whether or not such tangible personal property belongs exclusively to the applicant; and
(f) If such tangible personal property does not belong exclusively to the applicant, the names and addresses of the other owners. In addition, the application shall contain a space for a written acknowledgement by the applicant that the applicant is fully aware of the provisions of this chapter, that the applicant agrees to abide by such requirements, and that the applicant understands that he or she may be penalized under the provisions of Section 751.99
for failure to comply with the requirements of this chapter.
