§ 114.04 LICENSE APPLICATION; CONTENTS.
   (A)   A person desiring to render emergency wrecker service shall file with the Chief of Police, a written application which is signed by the applicant or his or her authorized representative.
   (B)   Each application shall contain the following information:
      (1)   Trade name, address and telephone number;
      (2)   Number and types of wreckers to be operated;
      (3)   Name, address and telephone number of the owner of the emergency wrecker company;
      (4)   Evidence of insurability pursuant of § 114.06(A)(3); and
      (5)   Agreement to comply with the requirements of this section.
