Whenever an employee of the City is injured in the performance of any of his duties, the head of the division by whom he is employed and who had supervision over him in the performance of the particular duties which resulted in his injury, shall fill out the proper form or forms prescribed by the Bureau of Industrial Relations, shall have them properly approved by the director of his department, and all papers shall then be forwarded to the Director of Finance for transmission to the proper office in Columbus.
