1717.03. Food service operations.
   (a)   The Director of Health is authorized to collect license fees pursuant to Ohio R.C. 3732.01 to 3732.99 and Ohio Administrative Code Chapter 3701-21.
   (b)   The following license fees are established pursuant to subsection (a) above, and do not include state certification fees collected by the department for remittance to the Ohio Department of Health.
   Fees
      (1)   Temporary food service operation     $94.00
      (2)   Mobile food service operations
         A.   Mobile food service operations which operate from and return daily to a mobile food service operation commissary, a licensed food service operation, or a similar place regulated by a governmental entity.   258.00
         B.   Mobile food service operations which do not return daily to a mobile food service operation commissary, a licensed food service operation, or a similar place regulated by a governmental entity.   258.00
      (3)   Vending food service operations per vending machine location.    26.55
         A.   This fee shall increase annually at license renewal, pursuant to the percentage increase in the Consumer Price Index for all urban consumers.
      (4)   Non-commercial food service operations fee shall be assessed on the basis of risk factors as follows:

      (5)   Food service operations fee shall be assessed on the basis of risk factors as follows:
