151.03 VETERANS MEMORIAL.
   (a)    The Committee shall determine if the following requirements have been met by the individual whose name is being requested to be placed on the Veteran Memorial located on the Tallmadge Circle:
      (1)   Individual must have served in a military action or conflict as declared by the President of the United States and/or Congress.
      (2)   Death of the individual is due to service while on active duty; and
      (3)   Based on military records, the individual's home address is Tallmadge, Ohio while serving on active duty.
      (4)   Based on military records, if the individual is listed as KIA, a star will be placed in front of the individual's name.
      (5)   If applicable, discharge from active duty must have been an "Honorable Discharge".
   (b)   The following procedural steps must be made for consideration of the request:
      (1)   All requests must be submitted in the form provided by the Veterans Committee, which shall include at least the following: name, military service number, rank, date of service, awards and decorations, place of entry into service (Home of Record), separation location (home town), date of death, geographic location of death, and place of burial.
      (2)   A copy of the candidate's military record and papers must be submitted which at minimum includes Discharge Form DD214 or an equivalent, and any other supporting documentation as requested by the Veterans Committee.
   (c)   The following definitions apply to this section:
      (1)   "Discharge Form DD214" is the Discharge Form DD214, or equivalent that are the official discharge papers created by the US Department of Defense.
      (2)   "Active Duty" is serving full time in a unit branch of the United States military.
      (3)   "Killed in Action" (KIA) is when an individual's death occurred during actual combat engagement by the enemy or battle field conditions as indicated on the DD214, or equivalent.
      (4)   "Home address" is the home of record at the time of entry into active service as indicated on the Discharge Form DD214, or equivalent.
