721.04 METHOD OF APPLICATION.
   (a)   Each specialty contractor shall make written application for registration or applicable registration renewal at the Building Department on forms available in that office and additionally shall provide:
      (1)    The applicant shall provide all information required by the Director of Public Service on the application form; and
      (2)    The applicant shall pay an initial registration fee of one hundred dollars ($100.00) and an annual renewal fee of fifty dollars ($50.00); and
      (3)    The applicant shall continuously hold or shall have assigned a valid and unexpired license issued pursuant to Ohio R.C. Chapter 4740 and file satisfactory proof thereof; and
      (4)    The applicant shall file with the application and all renewals a current certificate of insurance and maintain same during the registration period providing for general liability insurance in the minimum amount of three hundred thousand dollars ($300,000); and
      (5)    The applicant shall file with the application and all renewals a copy of a current Certificate of Coverage (Form DP22) or other satisfactory proof of coverage from the Ohio Bureau of Workers' Compensation; and
      (6)    The applicant shall file with the application and all renewals a valid and current Certificate of Registration issued by the City of Tallmadge Tax Administrator.
   (b)    Each and every other contractor shall make written application for registration at the Building Department on forms available in that office and shall additionally provide:
      (1)    All items listed in sections 721.04 (a) (1), (2), (4), (5), and (6), above; and
      (2)    Satisfactory completion of a test administered by the Building Department by passage with a grade of not less than seventy-five percent (75%).
