1020.03 FEE; INSURANCE.
   Such application shall be accompanied by a fee of twenty-five dollars ($25.00) with such further deposit as the City Engineer may specify as necessary to defray the cost and expenses of investigation and inspection of the work by him, as hereafter provided, and to restore the surface of the street to its former condition after the performing of any acts or work referred to in Section 1020.01. Such restoration shall include the removal of refuse, debris and dirt deposited on the paved surface of the street, driveways and sidewalks during the performance of such acts or work.
   The application shall also be accompanied by a public liability insurance policy or policies issued by an insurance company licensed to do business in the State, insuring the Municipality as well as the person doing the work, in form approved by the Law Director and in such sums as the City Engineer shall deem necessary, but in no event less than one hundred thousand dollars ($100,000) - three hundred thousand dollars ($300,000) personal injury liability and twenty-five thousand dollars ($25,000) property damage liability, conditioned to indemnify, defend and save harmless the Municipality from any debt, claim, suit, action or demand by any person or entity caused by or arising out of the work for which permission is sought under such application. The insurance policy or policies shall be in force when the acts or work are commenced and shall remain continuously in force while the work or acts are being done.
