151.04 HOSPITALIZATION INSURANCE.
   Each full time employee now or hereafter participating in the group hospitalization insurance offered to municipal employees who are not covered by a collective bargaining agreement shall be required to contribute to the premiums for such insurance according to the following formula:
   (a)   Ten percent (10%) of the premium per pay period for family or single coverage; calculated by taking the monthly premium multiplied by 12 and divide the result by 26 pay periods for most employees. Take ten percent (10%) of the result to calculate the deduction per pay.
   (b)   For officials paid monthly, take ten percent (10%) of the monthly premium to calculate the deduction per pay.
   (c)   Each bi-weekly employee's contribution will be capped at sixty-five dollars ($65.00) per pay for family coverage and thirty-five dollars ($35.00) per pay for single coverage as of September 1, 2009. The cap will be increased by five dollars ($5.00) per pay in each new year, effective on September 1 st of each year.
   (d)   Each monthly employee's contribution will be capped at one hundred forty-one dollars ($141.00) per pay for family coverage and seventy-six dollars ($76.00) per pay for single coverage as of September 1, 2009. The cap will be increased by eleven dollars ($11.00) per pay in each new year, effective on September 1 st of each year.
