945.06 HEARING.
   (a)   Information concerning the procedure to follow to request a hearing before the Utility Manager is provided on the back of the regular monthly bill. A hearing may be requested when there has been an irreconcilable failure of agreement between the utility office staff and the consumer concerning payment of account. The hearing date must be within seven business days of receipt of the request. The customer must pay the current portion due on the utility account until the hearing process concerning the delinquent portion is resolved to avoid further delinquent charges.
   (b)   The request for a hearing:
      (1)   Shall be submitted in writing to the Utility Manager within two (2) business days of the due date on the disconnect notice.
      (2)   Shall state the issue to be addressed which shall include but not be limited to:
         A.   Disputes as to amount due
         B.   Medical hardship
         C.   Financial hardship
         D.   Other reasonable cause.
   (c)   The hearing committee shall be comprised of the Utility Manager and include but not be restricted to the Mayor or his appointer, or Service Director, or his appointer. If all the above steps cannot be resolved with the hearing committee, the final step would be an appearance before the utility committee.
