Any person desiring a license for any such vehicle as described in § 70.01 above and within 30 days after acquisition of that vehicle or becoming a resident of the village, shall apply therefor to the Village Clerk and shall give the Clerk the following information:
(A) The name of the applicant;
(B) Address (street and number);
(C) Number of the automobile, motorcycle or motor truck for which the license shall be desired;
(D) Name of maker; and
(E) Style of body.
