862.03 APPLICATION FORM.

   (a)   Applicants for a solicitor's permit under this chapter must file with the City Clerk a sworn application in writing on a form to be furnished by the City Clerk, which shall give the following information:

      (1)   Name and description of the applicant;

      (2)   Address (legal and local);

      (3)   A brief description of the business;

      (4)   If employed, the name and address of the employer and description of relationship;

      (5)   Dates of solicitation; and

      (6)   If a vehicle is used, a description of same, together with the plate number.

   (b)   Any person or entity denied registration may appeal to the City Council and Council's decision shall prevail.
