840.06 RESTRICTIONS ON ISSUANCE OF LICENSES.
   (a)   No license required in this chapter shall be issued to any person who is not a resident of the United States.
   (b)   No license shall be issued to any person who has been convicted of violating any Federal statute or any State law or statute that has been designated as a felony.
   (c)   An application for such a license shall contain the following information:
      (1)   The name, address, age and date and place of birth of the applicant;
      (2)   Prior convictions of the applicant, if any;
      (3)   The place where a machine or device is to be displayed or operated and the business conducted at that place; and
      (4)   A description of the machine to be covered by the license, its mechanical features and the name of the manufacturer.
   (d)   No license shall be issued to any applicant unless he or she is over twenty-one years of age.
