1410.03 PERMIT APPLICATION, ISSUANCE, TERM, RENEWAL AND FEE.
   (a)   Every person who operates or desires to establish a child day care home or facility shall apply to the Building Commissioner for a permit on such form as he prescribes. The permit shall be renewed annually and a fee of twenty-five dollars ($25.00) shall be paid for each permit or renewal thereof.
   (b)   The Building Commissioner and the Fire Chief and their designated representative(s) are hereby authorized to conduct an inspection of all child day care homes and appurtenant structures at least once every year according to a schedule established by them. It shall be the duty of the owner or agent to cause the entire structure to be accessible for inspection at the time arranged, which shall be during regular business hours.
   (c)   Upon filing, a copy of the application shall be forwarded to the Fire Department for inspection of the premises under the Fire Prevention Code and all other applicable laws or codes.
   (d)   Concurrently, a copy of the application shall be forwarded to the Traffic Commissioner for comment on any traffic, safety or parking concerns.
   (e)   The Building Department, upon being satisfied that all requirements are complied with, shall issue a permit in such form and manner as prescribed from time to time by the Building Commissioner, including at least the name of the property owner and the name of the provider.
   (e)   Appeals from the refusal of the Building Commissioner to issue a permit, pursuant to Section 1410.04, may be made to the Zoning and Building Standards Board of Appeals as provided in Section 1411.07.
   (f)   The permit shall be valid only for the provider, at the address designated in the permit, and the permit shall be posted in a conspicuous place at the subject premises. The permit shall not be transferable.
