1150.05 APPLICATION, ISSUANCE AND RENEWAL OF MASSAGE TECHNICIAN LICENSE.
   (a)   A person intending to perform services as a massage technician shall make application to the Commissioner of Building upon such forms as may be prescribed by the Commissioner. Each application shall be nonrefundable. The application shall contain the following information:
      (1)   Name, residence address and social security number of the applicant;
      (2)   The name of the massage establishment owner or operator for whom the massage technician is employed;
      (3)   A physical description setting forth the applicant's weight, height, hair color and color of eyes;
      (4)   A recent two-inch by two-inch photograph of the applicant and a set of fingerprints which may be obtained from the Police Department;
      (5)   A certified copy of the applicant's birth certificate which evidences that the applicant is at least eighteeen years of age;
      (6)   The business, occupation and employment of the applicant for a three-year period preceding the date of application;
      (7)   Whether the applicant has been convicted of, plead no contest to, or suffered a forfeiture on a bond involving a charge of committing any crime, except minor traffic violations, within the last five years. If the answer is in the affirmative, a statement must be made giving the place and the court in which such conviction, plea or forfeiture was had, the specific charge under which the conviction, plea or forfeiture was obtained, and the sentence imposed as a result thereof;
   (b)   Upon application, the City shall conduct an investigation into the information contained on the application. The City shall issue a license to engage in the business of or be employed as a massage technician to the applicant unless the Commissioner of Buildings finds that the applicant has been convicted within the last five years of a felony; any offense involving sexual misconduct with children; keeping or residing in a house of ill fame; solicitation of a lewd or unlawful act; prostitution; pandering; or any crime involving dishonesty, fraud or deceit.
   (c)   A massage technician license issued pursuant to this chapter shall terminate at the expiration of one year from the date of its issuance.
   (d)   A massage technician license issued pursuant to this chapter may be renewed by the filing of a renewal application by the current holder of the license with the Commissioner of Buildings upon such forms that he may prescribe not later than thirty days prior to the expiration date of the license. A renewal appliation shall be accompanied by a renewal fee in the amount of fifty dollars ($50.00).
   (e)   The issuance of a license 'pursuant to this section shall not exempt the applicant from obtaining a massage establishment license pursuant to this chapter in the event that the applicant intends to operate a massage establishment.
