Any person desiring to secure a license to operate one or more vehicles for public hire, as required by Section 1131. 01,, shall make a written application therefor to the Director of Public Safety. A fee of twenty-five dollars ($25.00) shall accompany the written application to cover the cost of investigating the application and the advertising of notice for the public hearing hereinafter required.
Applications shall set forth the following:
(a) The full name, age and residence of the applicant. If the applicant is a partnership, the full name, age and residence of each partner, and if the applicant is a corporation, its principal address and the full names of its principal officers.
(b) The name under which the applicant proposes to do business.
(c) The number, type, markings and capacity of each vehicle to be used and the names of the drivers or operators thereof with such information as will show such drivers or operators can or do comply with the provisions of Section 1131. 08.
(d) A schedule of fares or rates to be charged and the method of changing same.
(e) That the applicant is capable of and will carry adequate liability insurance and bond as hereinafter provided.
(f) Such other information as the Director of Public Safety shall desire.
