Application for a license to display a mechanical amusement device or mechanical musical device must be made to the Mayor upon such forms as he shall prescribe. The application must be made by the owner or proprietor of the business or place at which the device or vending machine is to be displayed.
The application shall state the name of the owner of the place or business, the address of the place for which the license is applied, the residence of the owner, the serial number and name of the manufacturer of the mechanical amusement device or mechanical musical device, the owner of the same, the name and address of the distributor of the same, the nature of the business in conjunction with which the mechanical amusement device or mechanical musical device is to be displayed, and such other further information as may be required.
