931.02 USE PERMIT APPLICATIONS.
   Application for use of the Municipal Center Building shall be made to the Mayor, on forms furnished by the City. No application shall be accepted unless the applicant is a nonprofit organization or whose members at least two-thirds reside in the City. It shall be signed by the person or officer in charge, and if he is not a resident taxpayer of the City, it shall also be , signed by a resident taxpayer who shall be responsible for the proper use of the accommodations and facilities. The application shall contain the following information:
   (a)   The name and address and, if a corporation, the names and addresses of the principal officers.
   (b)   The date and hours of requested use.
   (c)   Designation of accommodations and facilities desired.
   (d)   Purpose of the requested use and of the organization.
   (e)   Maximum number of people expected.
   (f)   The amount of any charge being made by the organization.
   (g)   Items to be sold and the amount of each.
   (h)   Total membership and total who are residents of the City.
   Except as may be otherwise provided by Council, applications shall be submitted at least two weeks prior to the desired date of use, and Council Chamber may not be used by any one organization more often than once a month.
