Any person who proposes to subdivide a lot, tract or parcel of land shall submit an application for the approval of the subdivision. Such application shall be in writing and shall identify the tract of land proposed to be subdivided, describe the proposed subdivision and the purpose thereof and be signed by the owner of record of the tract proposed to be subdivided. Such application also shall state whether the subdivision is proposed to be effected by a plat or by a deed without a plat. Such application shall be accompanied by a written opinion of an attorney at law, licensed to practice under the laws of the State, stating the ownership of record of the tract proposed to be subdivided.
