7-2-2: APPLICATION FOR APPROVAL AND PERMIT:
   A.   An applicant seeking a permit to construct, alter, cut, grade or excavate the surface or the support for the surface of any street, road, highway, parkway, curb or curb cut shall file with the village board a written application, on eight and one-half inch by eleven inch (81/2" x 11") paper, which shall include:
      1.   Name and address of the owner or agent in charge of the party doing the work.
      2.   Location of the work areas.
      3.   Explanation of why the proposed cut, alteration or excavation is needed.
      4.   Explanation of alternatives considered with regard to the proposed cut, alteration or excavation and why the alternatives were rejected.
      5.   Date the cut, alteration or excavation is proposed to occur and the date on which the repair will begin and be completed.
      6.   The specifications of the cut, alteration or excavation proposed, including the length, width and depth of the area.
      7.   A map of the work area, on eight and one-half inch by eleven inch (81/2" x 11") paper, indicating the names of the surrounding streets, roads or highways, the location of the proposed cut, alteration or excavation and an arrow indicating which direction is north.
   B.   Each page of the applicant's written proposal shall be dated and signed by the applicant. The village board may request such other information as it determines reasonably necessary to the determination of whether approval should be granted and a permit issued hereunder.
