The application shall be subscribed to by the applicant in his own handwriting. It shall have attached thereto as a part thereof a certificate by a local physician procured at the applicant’s expense reciting that, within ten days prior thereto, such physician has given the applicant a careful physical and mental examination, and that he has found such applicant physically and mentally fit to operate as the driver of a taxicab on the streets of the city, that such applicant has met the specifications of KRS 186.411 with regard to seizure disorders, and that such applicant is otherwise free of any of the afflictions enumerated in § 118.05.
