(A) The City Clerk/Tax Assessor shall not grant any license for the use, operation, or possession of any machine as described in § 115.01.
(B) The City Clerk/Tax Assessor is further directed not to issue any license for a machine or device similar to those described under § 115.01 unless the applicant files with the application for a license an affidavit stating that the applicant does not have a federal gaming stamp for the use of the machine and further that the machine does not fit the description of those machines prohibited under § 115.01.
