719.03 LICENSE APPLICATION.

   Each applicant for a license to engage in the business of secondhand and precious metal dealer shall file with the Safety-Service Director's office a written application upon a form prescribed by the Director and signed by the applicant. Such application shall state:

   (a)    The names and residences of the applicants and the type of business entity of the applicant; and

   (b)    The premises where such business shall be located.
