§ 153.050 PROCEDURES AND REQUIREMENTS FOR SUBMISSION.
   (A)   Should the subdivider desire to have a concept plan review, the subdivider shall submit ten hard copies and one electronic copy (PDF) of a concept plan to the Development Review Committee, at which point, within 15 days after the request, the county will schedule a meeting to review the concept plan and give initial feedback.
   (B)   The plan shall include the following items:
      (1)   Municipal annexation request form;
      (2)   The proposed name of the subdivision;
      (3)   A vicinity plan;
      (4)   A proposed lot and street layout, indicating general scaled dimensions of lots to the nearest foot;
      (5)   (a)   A description of the type of water system proposed; also, documentation of water rights and of historic water use; and
         (b)   When private wells and on-site adsorption sewage disposal systems are proposed, a description of how each proposed lot will conform to the standard protection radius around the well-head (refer to § 153.126).
      (6)   A description of the type of sewer or sanitary waste system proposed;
      (7)   A description of the method of financing improvements and a statement concerning the timing of improvement installation, specifically improvements that are to be installed following the recording of the final plat (refer to §§ 153.147 and 153.148);
      (8)   The acreage of the entire tract proposed for the subdivision, including plans for maintenance and ownership of remaining non-buildable lands as required in Chapter 154 (refer to §§ 153.165 through 153.167);
      (9)   Geologic hazards within the tract; and
      (10)   Letters of feasibility from the appropriate health officers of the proposed water and sewerage systems, including irrigation, canals and ditches, necessary to meet the requirements of this chapter, the local health officer and the State Department of Environmental Quality, Central Utah District.
