(A) Any employee who is subjected to major discipline may, within ten calendar days after being notified by the City Manager of the imposition of such major discipline, file with the Secretary a written demand for an investigation.
(B) Simultaneously with the filing such demand, the accused shall cause to be delivered to the City Manager personally or mailed, by certified mail addressed to the City Manager at City Hall, the following:
(1) A written response to the charge(s) contained in the accusation;
(2) Names of the witnesses who will be called on behalf of the accused at the hearing before the Civil Service Commission, together with a general statement concerning the nature of their testimony; and
(3) Copies of the documents to be introduced on behalf of the accused.
