745.05 APPLICATION FORMS,

Applications for all permits required hereunder shall be filed with the Issuing Authority and shall be accompanied by the requisite fee, if any. The Issuing Authority shall prescribe the form of application and request such information as is necessary to evaluate and act upon the application.
