713.03 APPLICATION FOR LICENSE.
   (a)   Every person, society, club, firm or corporation desiring to open or maintain a bowling alley must make application to the Housing, Planning and Zoning Officer for a license therefor. Such application shall be in the form prescribed by the Housing, Planning and Zoning Officer.
   (b)   No license shall be granted to a person who is not a citizen of the United States.
   (c)   Every applicant for a license shall file with the Housing, Planning and Zoning Officer, a written application stating the full name and address of the applicant or firm, the full names of all parties interested, or if an association or firm, the full names of all parties and their addresses. If the applicant is a club, society or corporation, the applicant shall submit a complete list of the officers of the club, society or corporation, with their names and addresses, and shall also give the state in which the club, society or corporation is organized and the names of one or more persons whom the club, society or corporation desires to designate as its manager or managers, with their addresses. The application shall also state the following:
      (1)   The premises where the bowling alley is to be conducted.
      (2)   Whether the applicant or manager is a citizen of the United States.
      (3)   Whether the applicant has ever been engaged in operating a bowling alley and when, where and how long in each place operated within the past five years.
      (4)   The name of the owner of the premises in which the bowling alley is located and the complete address of the owner. The application shall be signed by the applicant or applicants or in the case of a club, society or corporation, the application shall be signed by the manager or any of the officers.
