(a) Every person desiring to open, conduct, operate or maintain a public billiard room or poolroom shall file with the Housing, Planning and Zoning Officer a written application stating his full name and address, or if more than one person or an association or firm, the full names of all parties interested and their addresses. If the applicant is a club, society or corporation, the application shall contain a complete list of the officers, their names and addresses, and the names and addresses of one or more persons whom it desires to designate as manager or person in charge.
(b) The applicant shall also state the following:
(1) The premises and address where the poolroom is to be conducted, and the number of tables proposed to be operated.
(2) The age of the applicant in case of individuals, and the ages of the manager and officers in case of a club, society or corporation.
(3) Whether the applicant is a citizen of the United States.
(4) Whether the applicant has ever engaged in operating the kind of business for which the license is sought, and when, where and how long in each place within five years then last past.
(5) The name of the owner of the premises in which the business is to be located and his complete address.
(c) The application shall be signed by the applicant, or in the case of a club, society or corporation, by its manager or any of its officers.
