§ 153.026 SUPPLEMENTARY INFORMATION; PLAT CONTENTS.
   The following information shall be in addition to the requirements of § 153.025 supplied:
   (A)   Statement of proposed use of lots, giving type and number of dwellings units, and type of business or industry;
   (B)   Description of proposed covenants and restrictions;
   (C)   Description of proposed zoning changes;
   (D)   Typical sections and complete profiles of streets, and other related improvements, to be constructed in the proposed subdivision; and
   (E)   In a letter accompanying the request for approval of the preliminary plat, the subdivider shall state the type of sewage disposal treatment he or she proposes to use. If, other than a treatment plant, it shall be accompanied by a letter from the Environmental Protection Agency, whichever is appropriate, stating what type of sewage disposal will be approved for the soil conditions encountered in the area of the proposed subdivision. A central sewage treatment plant and a central water system shall be constructed by the subdivider when deemed necessary by the Village Planning Commission, the appropriate Health Officer, or the State E.P.A.
