Such applications shall contain the following information and statements:
A. Name of the applicant;
B. Nature of the event;
C. The date or dates and hours when the proposed special event is to take place and location of the event;
D. A brief statement describing how the special event is to be conducted and in what manner it will be beneficial to the mall and the community;
E. A statement that the applicant agrees to abide by all regulations governing the use of such mall and all conditions which may be imposed upon such use by the city manager in granting of the permit;
F. Such other information which the city manager may reasonably require. (Prior code § 51.03.303)
