10-2-2: APPLICATION AND PERMIT TO CONNECT TO WATER SYSTEM:
   A.   Application To Connect:
      1.   Any person desiring to make any connection with the water system or plant or use water therefrom shall first make application to the Director of Public Works for that purpose upon a blank form to be furnished by the Director or Village Engineer.
      2.   Every applicant shall state specifically and in detail the place, location and kind of connection appendages sought, the purpose for which the water is to be used and all the plumbing or plumbing work in any manner or way connected therewith.
      3.   Applications for water service must state fully all the purposes for which it is required, and when paying the charges therefor, the parties must frankly and without concealment answer all questions put thereto relative to its consumption. The applicant shall, in such application, agree that in consideration of the insertion of the tap, he will pay or cause to be paid the assessment levied on the premises for the water pipe that is proposed to be tapped and all water rates or rents to accrue against the premises and that he will faithfully comply with the rules established in this chapter and all ordinances of the Village relative to the use of water. All applications for water must be made in all cases by the owner of the premises, or his duly authorized agent, for which water is sought to be supplied.
   B.   Compliance With Provisions: Every person so making the connection with the water system or plant or using water therefrom shall make the connection and use the water and maintain the same subject at all times to the provisions and conditions of this Code or the ordinances of the Village in force or to become in force at any time during the period the connections are maintained or the water is used and shall in all respects conform to the provisions and conditions of this Code or ordinances of the Village. The Village Plumbing Inspector must be there when tapping into any Village water main.
   C.   Issuance And Contents Of Permit:
      1.   When the applicant hereunder has complied with all the provisions of this Code or the ordinances of the Village and any rules or regulations of the Board of Trustees and paid to the Village Collector all of the charges connected with securing water service, a permit shall then be issued by the Director of Public Works authorizing the connection to be made, and specifying the size thereof and the location of the proposed tap and shutoff box.
      2.   The permit shall show the location of the tap and the shutoff box, size, length and weight of pipe and shall be endorsed by the Director showing that he has inspected the tap before the trench was refilled, and the tap and shutoff box are in the places specified, and in accordance with the requirements of this Code or the ordinances of the Village and the rules and regulations of the Board of Trustees, and the kind, location and size of tap service pipe and shutoff box shall then be entered upon the map or profile kept for that purpose as herein provided.
