10-2-3-5: FORM AND CONTENT:
The final plat shall be accurately drawn in ink on tracing cloth or mylar drafting film, in a manner that clear and legible transparent or contact prints or photostatic copies can be made. The final plat shall be at a scale of one hundred feet to the inch (100' = 1") on sheets not to exceed twenty four inches by thirty six inches (24" x 36"). Where necessary, the plat may be drawn on several sheets accompanied by an index sheet showing the entire subdivision. The final plat shall show the following:
   A.   Name of subdivision.
   B.   Legal description of property encompassing the subdivision. (Ord. 272, 3-20-78)
   C.   Names and addresses of the owner, owner of record (including the names and addresses of all beneficiaries of any land trust, accompanied by a certified copy of the trust agreement and all amendments thereto), the subdivider, and designer who prepared the plan. The proof of ownership shall be in the form of a certified copy of a title insurance policy or a tract search, supplied by a company in the business of investigating land titles, no more than sixty (60) days old at the time of said filing. In the event the title appears in the name of a land trust, the trustee or a beneficiary thereof shall identify each beneficiary of such land trust by name and address and define his interest therein.    D.   Scale, one inch to one hundred feet (1" = 100') shown graphically.
   E.   Date.
   F.   North point.
   G.   Boundary of plat, based upon an accurate traverse, with angular and lineal dimensions.
   H.   Exact location, width and name of all streets within and adjoining the plat, and the exact location and widths of all crosswalkways.
   I.   Distances to the nearest established street lines or official monuments (not less than 3) which shall be accurately described on the plat.
   J.   Township and section lines and Municipal boundaries accurately tied to the lines of the subdivision by distances and angles.
   K.   Radii, internal angles, points of curvature, tangent lengths, and bearings and lengths of all arcs.
   L.   All easements provided for public services, drainage and utilities.
   M.   Layout, numbers, dimensions and size of lots, in square feet. When lots are located on a curve, the lot width at the building setback line shall be shown. Street address for each lot.
   N.   Accurate location of all monuments which shall be concrete with iron pipe cased in center. A minimum of two (2) such monuments shall be set at the opposite extremities of the subdivision. Iron pipe stakes, a minimum length of twenty four inches (24"), shall be set at all lot corners, at each end of all curves, at all angle points in any line, at all angle points along a meander line, the points to be not less than twenty feet (20') back from the normal water elevation of a lake or from the bank of a stream, except that when such corners or points fall within a street, or proposed future street, then such pipes shall be set at such actual corners. All U.S., State, County or other official bench marks, monuments or triangulation stations in or adjacent to the property shall be preserved in precise position.
   O.   Lot or lots to be dedicated or reserved for public use, with the purposes indicated thereon, and of any lot or lots to be reserved by deed covenant for common ownership and use of all property owners.
   P.   Building setback lines accurately shown with dimensions.
   Q.   Protective covenants which shall be lettered on the final plat, or on an attached document in form for recording. (See Form 6 as shown in Chapter 6 of this Title, and reference in Section 10-3-4C of this Title.)
   R.   Listing of all public authorities having jurisdiction thereof.
   S.   Owner's (and, if required, any mortgage holder of record) certificate to the effect that he has caused the land described to be subdivided along with a notary certificate of owner's signature.
   T.   Need to include notary certificate.
   U.   President and Board of Trustees' certificate approving and accepting the plat attested by the Village Clerk.
   V.   Recorder's certificate certifying that the final plat is recorded in the County of Lake.
   W.   Surveyor's certificate to the effect that he has surveyed and subdivided the property and the plat represents a true and accurate representation of said survey and subdivision.
   X.   Plan Commission certificate approving the plat.
   Y.   County Clerk certificate to the effect that there are no taxes against any land included in the plat.
   Z.   Special assessment.
   AA.   Village Engineer's certificate to certify that all of the improvements meet the standards of all Village ordinances, certifies that the final plat conforms with the preliminary plan and includes the proper easements.
   BB.   Village Attorney's certificate certifies that the improvement security is in the proper form and amount, checks the final plat and all accompanying documents.
   CC.   Certificate of easements for all public utilities and such other disclosures.
   DD.   Such other certificates as may be required by Illinois State Statutes.
