3-4-10: APPLICATION AND CONTENTS:
   A.   The application for a license required by this chapter shall be on such form as may be prescribed by the village president or his designee. The application may require the following information to be submitted:
      1.   The location of sale or other means by which raffle chances will be sold or issued;
      2.   The time period during which raffle chances will be sold or issued;
      3.   The time of determination of winning chances;
      4.   The name and address of the organization;
      5.   The type of organization;
      6.   The number of years that the organization has been in existence;
      7.   The date and place of incorporation or formation of the organization;
      8.   The number of members in good standing in the organization;
      9.   The name of the president or chairperson of the organization;
      10.   The name of the raffle manager;
      11.   The cumulative retail value of the prizes;
      12.   The maximum value of each prize;
      13.   The maximum price to be charged for each chance sold;
      14.   The location or locations at which winning chances will be determined;
      15.   A sworn statement attesting to the nonprofit character of the prospective licensee organization signed by its presiding officer and secretary, if any.
   B.   In addition to the foregoing information, the application shall contain such other and further information as the village president or his designee may prescribe not inconsistent with law. (Ord. 07-8-11, 8-21-2007)
