An application for a gun shop license shall be made in writing upon a form provided by the Village, signed by the applicant, and shall set forth the following information pertaining to the prospective licensee:
(a) The name and address of the applicant. If the applicant is a corporation, the name and address of the Chief Executive Officer and the Secretary of the corporation is required. If the applicant is a partnership or other business entity, the names and addresses of all partners, managers, or directors are required.
(b) Evidence that the applicant possesses a Federal Firearms Dealer License, and any other license that may be required under State or Federal law to engage in the retail sale of firearms.
(c) If approved, the fee for a gun shop license shall be as set forth in Section 802.09 of the Code of Ordinances.
