§ 152.02 PERMITS.
   (A)   No mobile home shall be placed within the limits of the village as of the date of the passage of this chapter without first securing a permit. An application form for such permit shall be obtained from the Village Clerk and shall include the following information:
      (1)   Description of the mobile home including year, model and identification number;
      (2)   Lot size and square feet of the lot area;
      (3)   Underpinning shall consist of material manufactured specifically for that purpose;
      (4)   Sanitary sewer connection;
      (5)   A list of all defects in the exterior covering, including, but not limited to, missing coverings and windows, screens and doors;
      (6)   Pictures of the front and back of the mobile home;
      (7)   Date the mobile home shall be brought into village;
      (8)   Name of the owner of the real estate where the mobile home shall be placed;
      (9)   Name of the person owning the mobile home;
      (10)   Name of the persons occupying said mobile home;
      (11)   All wheels shall be removed from the mobile home; and
      (12)   Roof pitch.
   (B)   If the application is approved, the application permit shall be issued and shall be filed with the Village Clerk.
