Taps to the sewer may be made only by authorized authority personnel:
A. The owner or his agent shall make application on a form provided by the authority.
B. The permit application shall be supplemented by any plans, specifications, or other information considered pertinent in the judgment of the authority.
C. A permit and inspection fee for taps on sewer lines shall be paid to the authority at the time the application is filed.
D. All costs and expenses incident to the installation and connection of the building sewer shall be borne by the owner.
E. The owner shall indemnify the City from any loss or damage that may directly or indirectly be occasioned by the installation of the building sewer.
