The permit application shall be made on a form provided by the Village. All Applications shall include:
(A) A signed agreement stating that the operator will follow the process and requirements described in this chapter.
(B) A signed authorization from the property owner to allow placement of the box on the property.
(C) A signed acknowledgment of responsibility from the property owner and the operator for joint and several liability for violations of this chapter.
(D) Evidence that the nonprofit has been registered as a nonprofit organization within the State of Illinois.
(E) The name, address, email, website (if available) and telephone number of the box operator and property owner.
(F) A vicinity map showing the proposed location of the box and the distance between the site and all existing boxes within 1,000 feet of the proposed location.
(G) A site plan containing property boundary, location of the box, distance between box and property lines, dimensions of the box, photographs of the location and adjacent properties.
(H) A description of the proposed box locking mechanism.
(I) A maintenance plan for the box including pick-up schedule, litter and trash removal, graffiti removal, etc.
(J) Completion of and compliance with any and all other requirements required by the Code. (2021-19, 12-28-2021)
