9.40.050   Persons prohibited from providing services.
   Pima health system shall not allow any person to provide or continue to provide services as an attendant care worker, if any of the following occur:
      1.   The criminal history records check or the sworn statement required pursuant to Section 9.40.040 indicates that he is awaiting trial on, or has been convicted of, any offense listed in Section 9.40.040(A); or,
      2.   The prospective or current attendant care worker fails or refuses to submit fingerprints pursuant to Section 9.40.020; or
      3.   The prospective or current attendant care worker fails or refuses to submit the sworn statement pursuant to Section 9.40.040; or,
      4.   The prospective or current attendant care worker submits a false notarized statement regarding criminal offenses.
