Every application for a license to hold, operate or conduct an outdoor mass gathering shall be typewritten or printed, notarized, consist of an original and five copies, and contain the following information:
A. The date or dates and the hours during which the outdoor mass gathering is to be held, operated or conducted and during which the site will be held under the control of the applicant;
B. An estimate of the minimum and maximum number of customers, spectators, participants and other people expected to attend the outdoor mass gathering for each day it is held, operated or conducted, together with detailed information supporting such estimate:
C. The nature and purpose of the outdoor mass gathering and the nature and kind of entertainment, if any, which is to be a part of or presented during the outdoor mass gathering;
D. The applicant's name (including aliases, if any), age, residence address, main living or business address and telephone number shall be required and the name (including aliases, if any). age, residence address, mailing or business address and telephone number of the president, or vice-president(s), and secretary of the corporation shall also be required:
E. The name (including aliases, if any), age, residence address, mailing or business address and telephone number of the owner of the site upon which the outdoor mass gathering is to be held, operated or conducted. If the owner of the site is a partnership or corporation, the same information required of such entities in subsection D of this section shall be required in this subsection;
F. The exact location, size and shape of the site upon which the outdoor mass gathering is to be held, operated or conducted: and
G. The exact location, size and shape of the land to be used for automobile parking spaces if land other than the site is to be used for this purpose, and the name (including aliases, if any), age. residence address, mailing or business address and telephone number of the owner of the land to be used for automobile parking spaces if the owner is a person other than the person who owns the site. If the owner of the land to be used for automobile parking spaces is a partnership or corporation, the information required of such entities in subsection D of this section shall be required in this subsection.
