A. No person shall receive any immunization agent at the health department without paying the required fee except under one or more of the following conditions:
1. In the event of a public health emergency,
2. Upon proof of extreme financial hardship as set by Health Department sliding fee standards,
3. If so waived by the Health Director in the interest of community welfare, or
4. When children are receiving immunizations in accordance with A.R.S. §§ 36-673 and 36-692.
B. The director shall establish a fee schedule which is in compliance with the definition of a reasonable fee, as provided in Section 8.12.020(G), and such fee schedule shall be reviewed by the board of health to verify that such fees are reasonable and just and, upon approval by the board of supervisors, shall be adjusted.
C. Federal, state and commercial payer reimbursement guidelines will be followed. Payment by third party payers for vaccination fees set forth below is accepted as payment in full.
D. Immunization fees shall be:
1. International Travel.
a. Vaccines required or recommended for international travel will be provided to adults and children for the variable cost of each vaccine administered plus a fixed cost of fifteen dollars per vaccine per visit plus a consultation fee of forty dollars for adults nineteen years of age and older and twenty dollars for children eighteen years of age and younger. The type of vaccine to be administered will be determined by established protocols, best practice standards, and screening review protocols.
b. For vaccines Typhim Vi and Immune Serum Globulin (ISG) there is an additional ten dollar handling fee. For Yellow Fever vaccine there is an additional twenty dollar fee, which includes a handling and stamp fee.
c. A ten dollar fee will be charged for any duplication of international travel immunization record requested.
2. Immunizations (Non-International Travel).
a. Adult vaccines, will be provided for the variable cost of the vaccine plus a fixed cost of fifteen dollars per vaccine per visit. Subsequent vaccines, given the same day as the initial vaccine, will be given for the variable cost of the vaccine plus a fixed cost of ten dollars per vaccine. The type of vaccine to be administered will be determined by established protocols, best practice standards, and screening review protocols.
b. Children age eighteen years and under will not be charged for age appropriate vaccines. Vaccine administration costs will be billed to third party payers in accordance with the payers' rates. Vaccine costs may be charged to third party payers. Charges to third party payers will be consistent with the fair market value of vaccines.
c. Adult influenza vaccine will be provided for fifteen dollars.
E. A schedule of the total charge to the client for each vaccine will be conspicuously posted at all health department clinic sites which offer immunizations.
