Any person, firm or corporation desiring a license as a water line installer within the Municipality shall:
(a) Submit an application to the Manager upon the form provided;
(b) Provide a certificate from an insurance company licensed to do business in the State of Ohio that the applicant at the time of application carries a policy of public liability insurance with limits of one hundred thousand dollars ($100,000) and three hundred thousand dollars ($300,000) as affects injury to persons, and limits of fifty thousand dollars ($50,000) and one hundred thousand dollars ($100,000) as affects injury to property;
(c) Sign a statement included in the application that the applicant is familiar with the type of material required and the installation of water lines;
(d) Include a statement that the applicant is acquainted with the Manual of Accident Prevention in Construction and has read the same and shall follow the regulations set forth therein relative to the installation of water lines; and
(e) Agree to provide experienced operators and workers to carry out the practices of installing water lines.
