Any application for a preliminary plat or subdivision approval, or total or partial vacation of an existing subdivision, dedication of a street, grant of a right of way or easement for the benefit of the public shall be made on forms obtained from the city engineer or city clerk, and the accuracy of the data provided thereon certified thereto by the developer.
