§ 33.23 FORM OF GRIEVANCES AND SETTLEMENTS.
   (A)   Form of grievance. In reducing a grievance to writing, the writing should specifically state that the matter is a grievance under §§ 33.20 through 33.26 and should specifically include the following information:
      (1)   A complete statement of the facts, including the exact date and nature of the act or omission giving rise to such grievance;
      (2)   The identity of the grievant, of the party or parties alleged to have committed the act or omission giving rise to such grievance and of any witness to such act or omission;
      (3)   The specific provisions of the applicable chapter or of any applicable procedure, order, rule, regulation or policy developed or made by the city or any such employee's department head that are alleged to have been violated; and
      (4)   The remedy or relief requested.
   (B)   Form of settlement. The settlement of any grievance under §§ 33.20 through 33.26 shall be reduced to writing and signed by both the city and the grievant.
